Pre-Tournament Health Declaration

This tournament requires a health declaration before you compete. The information you provide is used exclusively by the tournament medical staff to ensure your safety on the day. It is never published and is never linked to any anonymous injury report.

   Fill in the form below — personal details, any relevant medical conditions, current medication and required declarations. It takes about 3–5 minutes.

   Submit once. After submission a confirmation email will be sent to your address. Keep it — you will need to show it at the medical check-in desk.

   Bring your confirmation email to check-in. Show it on your phone or as a printout to the medical officer at the desk. If a doctor approval is required, bring the original document. Your weight will be verified on-site.

   Compete at the tournament. No further action needed on your part during the event.

   After the tournament, the EKO Medical Committee will email you a link to the anonymous injury reporting form. It takes about 2 minutes to complete.

   Complete the form anonymously. Your email is used only to send the link — it is never stored alongside your answers.
