How to participate in injury reporting

This tournament uses email-only registration. No personal information is required now — just leave your address so we can send you the form link after the event.

   Register your email below — the only step required before the tournament. Your address will never be published or linked to your injury data.

   Compete at the tournament. No further action needed on your part during the event.

   After the tournament, the EKO Medical Committee will email you a link to the anonymous injury reporting form. It takes about 2 minutes to complete.

   Complete the form anonymously. Your email is used only to send the link — it is never stored alongside your answers.
